Click the link above to use Accurx, our online appointment and admin request system. You can use Accurx to submit medical and administrative requests.
It should not be used for emergency requests, so if you need immediate attention, please call 999 or use local emergency care providers.
Submitted forms will only be read during surgery hours and not on evenings, weekends or bank holidays.
Submitting Requests
There are two types of requests you can make through Accurx:
A Medical Request
- A new or ongoing health problem
An Admin Request
- Repeat prescriptions
- Fit (sick) notes
- Reviews
- Screening
- Vaccinations

How Requests Work
Accurx Patient Triage is an easy way to contact your GP practice for medical or admin help without waiting on the phone. You fill in a short online form (or staff can do it for you if you call or visit). This form goes straight to the clinical team, who look at every request.
Your request is then sorted by urgency for example:
- Urgent medical issues are seen by a clinician as soon as possible.
- Routine medical requests are booked for a future appointment, followed up with advice, or signposted to a different service if that’s more suitable.
- Admin requests (like sick notes or prescriptions) go to the admin team.
The practice team decides the best next step; this might be a call, text message, or an appointment in person or by phone.
Urgent appointments
To request an urgent appointment (Monday to Friday) during opening times:
- use the Accurx service
- phone us on 01908 872500 if you can’t use the online service
When you get in touch, we’ll ask what you need help with.
Routine appointments
To request a routine appointment in advance during opening times:
- use the Accurx online service
- phone us on 01908 872500 if you can’t use the online service
- use your NHS account (through the NHS website or NHS App) to book an appointment, screening test or vaccination
When you get in touch, we’ll ask what you need help with.
We will use your answers to choose the most suitable doctor, nurse or healthcare professional to help you.
Appointment times
Patients are given a choice of appointments throughout the day from 8:30am to 6pm, Monday to Friday
There is also a system in place for patients who need to be seen urgently, either by the duty doctor or the nurse practitioner.
Cancelling or changing an appointment
To cancel your appointment:
- Before your appointment you will receive a text message reminder with a link allowing you to cancel your appointment if you wish
- use your NHS account (through the NHS website or NHS App)
- phone us on 01908 872500 during opening times
- You can also cancel in person at the practice, but please let us know in advance so we can offer the slot to another patient.
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
If you need help with your appointment
Please tell us:
- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- if you need an interpreter
- if you have any other access or communication needs
