Registering with the surgery

To register with the surgery:

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

When you visit the surgery for the first time you will be asked to sign both forms to confirm that the details are correct.

Requirements

Anyone in England can register with a GP surgery. You do not need proof of address or immigration status, ID or an NHS number. Please ensure that you provide current contact details.

New patient checks

The practice needs all newly registered patients to consult with one of the Partners or a practice nurse within the first three months of registering. Medical records can take some time to arrive at the practice therefore it is important for the practice to obtain as much medical information as possible about newly registered patients.

Patients who are on regular medication, attending the hospital for ongoing treatment, under the age of 5 years or over the age of 75 years, will need to consult with a doctor when registering with the practice.

Named GP

All patients at our practice have a named GP to oversee their care. The named GP is largely a role of oversight. It reassures patients they have one GP who is responsible for their care.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
  • The named GP works with relevant health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient and is responsible for ensuring patients have access to a health check where applicable.

All patients who previously had Dr Hobza as their named GP have been allocated to Dr Kemp or Dr I Anson as their named GP to oversee their care.
