The Brief

To support One Ellesmere Port and its 10 GP practices as they embarked on a collaborative journey to deliver population health care.

The Objectives

Bring Partners and Practice Managers together to create a shared vision for health care across Ellesmere Port:

• Facilitate the appointment of a Council, Steering Group and leadership team for the network
• Develop a strategy to help the network engage with stakeholders
• Design branding and marketing tools for the network.

Approach

OneMedical Group delivered a series of interactive workshops including the  local authority, community nursing  team and HealthWatch. From managing the election process of the PCN Council and Steering Group, to chairing meetings and sharing best practice from across  the country, OneMedical Group was able to use its knowledge of primary care to shape the formation of the One Ellesmere Port PCN.
